Incomplete Grade Agreement
Graduate students and instructors in AES should use this form if requesting an incomplete grade.
Form instructions:
- Instructors should initiate the form by filling in the contact information below.
- Information that will be required on the form: student name, student ID number, instructor name, course in section number, term, and year.
- Instructors will detail the reason for the incomplete, the coursework that was completed, and the action required to resolve the incomplete grade. Instructors will then sign the form and click 'finish.'
- The form will be routed to the student for acknowledgment.
- The instructor will receive a copy of the form when completed.
