The forms on this page are strictly for instructors to fill out.

Grade Change Workflow/Form:

A Grade Change Workflow can be filled out on the MyCUInfo Teaching Tools Tab.  The Grade Change Workflow instructions can be found here.

Classroom Change Form:

The Classroom Change Form is used to inform the administrators (Terri, Megan, Karen) that there will be a classroom change.
You will need to know:

  • The course and section number i.e. PHIL XXXX - XXX
  • The date, day of the week, and time of the changed class
  • The building and room where the changed class will take place

Class Cancelled Form:

The Class Cancelled Form is used to inform the administrators (Terri, Megan, Karen) that class will be cancelled.
You will need to know:

  • Classroom number i.e. HLMS XXX
  • Class name i.e. Introduction to Philosophy
  • The course and section number i.e. PHIL XXXX - XXX
  • The date, day of the week, and time of the changed class
