Each day you come to campus, you are expected to fill out the Daily Health Form.
Individuals should self-screen daily before leaving home for any COVID-19 symptoms and complete the daily health questionnaire as directed. If you are displaying any of the symptoms or feeling ill, you should stay at home, consult with your primary care physician and notify your supervisor.
Cleaning Supplies Purchase Request
To ensure that the UMC is clean and safe upon reopening and to support our building partners, the UMC will be providing cleaning products for purchase. Bottles of cleaner and rolls of paper towels will be available to UMC tenants that are interested at the Reception Desk beginning August 3. Sets (one bottle + one roll of paper towels) will be available for $5.00 each.
Please complete the Cleaning Supplies Purchase Request Form to request cleaning supplies. An email confirmation will be sent with information about pick up. You will receive an invoice monthly from the UMC Business Office.
If you are interested in purchasing your own cleaning products, products can be found on the Marketplace in the COVID product section and/or through Staples.
Users that may request keys, include UMC staff, UMC tenants and vendors, student employees, RSOs, and contractors completing approved work in the building.
The UMC will contact the requestor if there are any questions regarding the need for keys being requested. All key requests must be approved by the department head.
Download and review the Travel Expenses Form prior to your business travel. Turn in the completed form within 10 days of your return.
