UMC Access Request

Please complete the following form to request room/building access for the University Memorial Center.

Please note:

Access may be requested by UMC staff, tenants and vendors; student employees; and contractors completing approved work in the building.
The UMC will contact you if there are any questions regarding the need for access being requested.
All requests must be approved by the department head. The UMC will issue the appropriate access to meet needs as identified below.
Please submit a separate request for each individual in your department in need of access.
A confirmation email will be sent to you when access has been granted and is available on your card.
