Create a Daily Health Questionnaire Icon On Your Smartphone

Below are instructions on how you can create a website icon on your smartphone. 

   Apple iPhone website icon shortcut 

1. Open the Safari browser. 
2. Navigate to the Health Questionnaire Form.
3. Press the Action Key located at the bottom of your screen. 
4. Press the "Add to Home Screen" button. 
5. Give your shortcut an appropriate name and press add. 
6. Your icon will now appear on your phone. You now have a quick link to the web site. 

   Android website icon shortcut 

1. Launch Chrome for Android.
2. Navigate to the Health Questionnaire Form.
3. Tap the menu button and tap Add to the home screen.  
4. Enter a name for the shortcut and then Chrome will add it to your home screen. 
5. The icon will appear on your home screen like any other app shortcut or widget, so you can drag it around and put it wherever you like.
