Those pursuing the FFD must develop a project in conjunction with a mentor at another institution in the Collaborative Preparing Future Faculty Network (COPFFN). A Mentorship Plan must be completed in order for the Graduate Teacher Program to endorse the student's placement and for the placement to be considered "sponsored" by the University of Colorado at Boulder. The student, employer, and the Graduate Teacher Program agree to assume these responsibilities for the duration of the student's placement.

This form signifies your department chair, academic advisor, TA supervisor/coordinator (if applicable), and selected Lead faculty mentor approve your participation in the Lead Network for your department.
