Completion of a daily health form is required for all faculty, staff and students each day you intend to be on the CU Boulder campus. Here are the five things you need to know about the form:

  1. It’s required of all students, staff and faculty members every day you plan to be on campus (both inside and outside). 
  2. It helps track (and thus limit) possible COVID-19 cases by asking questions about body temperature, symptoms, close proximity to someone who may have COVID-19 and COVID-19 tests results if applicable. People at risk for the illness may be asked to stay home or see a doctor. 
  3. It only takes a few seconds, and you can make it easier by creating a shortcut on your phone (and an app is in the works!). Set a daily reminder on your phone or calendar. 
  4. Knowledge is power in the fight versus COVID-19 and surveillance through questionnaires is a public health best practice. 
  5. All personal health and medical information will remain confidential. Once your form is complete you will get an immediate response about whether you should can come to campus or if you should stay home and contact your medical provider.
