This form is only needed if you received the following initial grades: D-, D, D+, C-, P, or P+. If you received an “F” in your initial completion of the course, you do not need to file this form. If you received a letter grade of C or higher on your initial attempt, you may not be eligible to repeat a class with VA funding.
