Receiving Campus Cash for Campus Culture Survey Participation

In order to receive Campus Cash for completing the Campus Culture survey, you will need to submit a W-8 BEN tax form (at the top of the second page).

• You must submit your completed W-8 BEN to ISSS via the Document Submission Form no later than December 10, 2021 to be eligible to receive Campus Cash.
