 COVID-19 Vaccine Requirement Ending - In accordance with state and federal guidelines, CU Boulder’s COVID-19 vaccine requirement for students, faculty and staff will end effective May 15, 2023.
Vaccinations and boosters continue to be strongly recommended as a safe, effective tool for minimizing the effects of COVID-19, including serious illness and death.
Vaccinations will continue to be required for students and staff working in CU Boulder health care facilities in alignment with federal/state guidance.

Immunization Requirement Overview: Fall 2023

All students must complete the following two immunization requirements. Students must remain compliant with the requirements throughout their time at CU Boulder.
 

Requirement 1: 

All students must submit proof of all required vaccinations or complete an exemption form.

Required Vaccinations:

  • Measles, Mumps and Rubella (MMR): Colorado State Law requires all students to submit proof of two (2) doses each of the measles, mumps and rubella (MMR) vaccination or proof of immunity (titers).
  • Meningitis (Men-ACWY): CU Boulder requires all students under the age of 23 living in university-operated housing to submit proof of the meningitis (Men-ACWY) vaccine given within the last five years and given after their sixteenth birthday. The meningitis (Men-ACWY) vaccine information must be completed by students living in a residence hall, Bear Creek, Athens Court, Athens North, Marine Court, Newton Court or Smiley Court.

Requirement 2:

All students, regardless of their vaccine status, must complete:

Required Forms:

  • Tuberculosis (TB) Risk Assessment and 
  • Immunization Questionnaire related to Meningococcal Disease and Human Papillomavirus (HPV) vaccine.

Required Vaccinations:

  • Meningitis (Men-ACWY) - only for students under the age of 23 living in university-operated housing

To complete your vaccine requirements follow the steps below:
 

Step 1:

Go to the MyCUHealth portal, and select the option for Students and Graduate Students. Login with your CU Identikey and password.

Step 2:

Click Immunizations in the left margin.

Step 3:

Follow page instructions to add required vaccine dates and submit proof of vaccination using the "upload tab/feature."

Please note: Students will not get a confirmation that proof of vaccination or vaccine exemption was successfully submitted. Overall status is available on the portal and will reflect Verified once all requirements have been completed and verified. The process of verifying records may take 7-10 business days.

Instructions to complete an exemption form

Students who cannot or who choose not to get a vaccine can complete an exemption.

  • Medical exemption: Medical exemptions must be signed by a licensed physician and include a date, medical license number, and a reason for the exemption. 

Note: As with all required vaccines, in the event of an outbreak, exempted persons may be subject to exclusion from CU Boulder and quarantine. The specifics may vary depending on the circumstances of the outbreak.

 

Step 1:

Login to the MyCUHealth portal with your Identikey username and password.

Step 2:

Click on the Immunizations in the left margin.

Step 3:

For a medical exemption: For a medical exemption: Obtain a completed signed Certificate of Medical Exemption form from your licensed physician. The form can also be found on the CDPHE vaccine exemptions website.

For a non-medical exemption for MMR and Men-ACWY: 

  • Option 1: Complete the Certificate of Nonmedical Exemption. This must be signed by a Health Care Provider licensed in the state of Colorado. Or ​
  • Option 2: Complete the Online Immunization Education Module. At the completion of the module, you will be able to complete an electronic form to obtain a Certificate of nonmedical exemption. Print and save the certificate to your computer.

 

Step 4:

For medical and non-medical exemption: Once your form is completed, login to the MyCUHealth portal. Click on the “Upload” button at the top and follow the instructions to upload your form as an Immunization Document.

Complete the required forms

All students, regardless of their vaccine status, must complete a:

Step 1:

Go to the MyCUHealth portal, and select the option for Students and Graduate Students. Login with your CU Identikey and password.

Step 2:

Click Forms at the top of the page. Select Required Immunization Questionnaire (the first link available). Answer both questions and hit the submit button.

Step 3:

Click Forms at the top of the page. Select the Required TB Risk Assessment (the second link available). Answer the presented question(s) and hit the submit button.
