Immunization Requirement Instructions

For Fall 2022, all immunization requirements must be completed by August 1, 2022. All students must complete the following vaccine requirements:

  • COVID-19 Vaccine: All new and returning undergraduate and graduate students must complete the COVID-19 vaccine requirement.
  • COVID-19 booster: To comply with CU Boulder’s updated COVID-19 guidance, members of the campus community are required to receive a vaccine booster shot five months after receiving the final dose of the original series.
  • MMR Vaccine: All new undergraduate and graduate students must complete the MMR vaccine requirement.
  • Meningitis Vaccine: All students under the age of 23 living in university operated housing must complete the meningitis vaccine requirement.
  • New Student Questionnaire: All new undergraduate and graduate students.

Providing proof of vaccination

To complete your vaccine requirements follow the steps below:
 

Step 1:

Go to the MyCUHealth portal, select “Students and Graduate students” and login with your Identikey username and password.

Step 2:

Click “Immunizations” at top of the page, a drop-down menu will appear. Select “Enter dates”.

Step 3:

Follow directions on the Immunizations page in the portal to add your vaccine dates and submit proof of vaccination.

Please note: Students will not get a confirmation that proof of vaccination or vaccine exemption was successfully submitted. Students will receive an email stating they have completed their requirement only when all vaccination requirements are complete. The process of verifying records and sending confirmation of receipt may take 7-10 business days.

How to complete an exemption

Faculty, staff and students who cannot or who choose not to get a vaccine can complete an exemption.

Note: As with all required vaccines, in the event of an outbreak, exempted persons may be subject to exclusion from CU Boulder and quarantine. The specifics may vary depending on the circumstances of the outbreak.

There are two types of exemptions:

  • Medical exemption: Medical exemptions must be signed by a licensed physician and must include a date and a reason for exemption.
  • Moral/Ethical exemption: A moral/ethical exemption can be requested based on moral or ethical beliefs as to what is right or wrong that are held sincerely with the strength of traditional religious views.

 

Step 1:

Login to the MyCUHealth portal with your Identikey username and password.

Step 2:

Click on the “Immunizations” button at the top and from the drop-down menu select “Enter dates.”

Step 3:

For a medical exemption: Click on the "Medical Exemption" link on the Immunizations page. Print and complete the medical exemption form. This form must be signed by a licensed physician.

For a non-medical exemption for MMR and Men-ACWY: Click on the "Non-Medical Exemption" link on the Immunizations page. In response to the question, “Are you a student submitting a Moral/Ethical exemption for COVID-19 and/or a vaccination exemption for MMR or Meningococcal?” select “Yes” and check the boxes for the vaccination exemption being submitted. Complete the remainder of the form AND one of the following two options if exempting from MMR OR Men-ACWY vaccines:

  • Option 1: Complete the Certificate of Nonmedical Exemption. This must be signed by a Health Care Provider licensed in the state of Colorado. Or ​
  • Option 2: Complete the Online Immunization Education Module.

 

Step 4:

For medical and non-medical exemption: Once your form is completed and signed by a physician, login to the MyCUHealth portal. Click on the “Upload” button at the top and follow the instructions to upload your form.
