Graduate Student Dental Insurance
The Graduate Student Dental Insurance Plan is available to students on full external fellowships or those with a graduate appointment of 20% or greater (RA/TA/GA or GPTI only), regardless of their medical insurance coverage (CU Anthem Gold SHIP or private insurance). Eligible students are automatically enrolled in the dental plan and cannot opt out; however, use of the plan is not mandatory, and students may choose to use their own outside dental coverage if preferred.
Students who are not in qualified degree programs or do not hold eligible appointments cannot opt in to this plan. For example, Law students, MBA students, and those in professional master’s programs who are not eligible for graduate appointments are not eligible for this plan.

Dental plan eligibility
The Graduate Student Dental Insurance Plan is available for students on fellowship or with a graduate appointment of 20% or greater regardless of their medical insurance plan (CU Anthem Gold SHIP or private insurance).
Dental plan enrollment
CU Boulder graduate students on fellowship or with a graduate appointment of 20% or greater are automatically enrolled in the Graduate Student Dental Insurance Plan once their tuition waiver is entered into Campus Solutions.
Notification of enrollment will be sent directly to students within 5 business days of the tuition waiver being entered. If students do not receive notification of their enrollment, they should first check with their employer to make sure the tuition waiver has been entered correctly and they meet the criteria for eligibility.
Please note: Students insured through the CU Anthem Gold Student Health Insurance Plan (SHIP) are eligible for a $150 annual credit by submitting a dental services invoice (exam, cleaning or X-ray) to studentinsurance@colorado.edu. For low-cost options for dental services, you can visit Low Cost Dental Options. Graduate students on appointment will be automatically enrolled in the Graduate Student Dental Insurance and are not eligible for the $150 credit.

Finding a provider
You can find an in-network dental provider by visiting anthem.com/find-care. Here’s how:
Step 1: Enter your Member ID prefix (BKJ) into the search menu at anthem.com/find-care. Click “Search”.
Step 2: Select your city, county or zip code in the top right search bar.
Step 3: Search for providers by:
- Entering a specialty, name or National Provider Identifier (NPI) into the search bar
- Selecting “Dental Professionals” option from the bottom menu
Step 4: Explore providers available in your city or county. Select “View Details” to view more information about a particular provider.
