ICT Review Process Forms

The following forms are to be completed by CU Requestors and Suppliers in the ICT Review process and ordering of any Information Communication Technology (ICT).

Please note: The forms listed below are now hosted on the OIT website to be integrated with ServiceNow. If you've filled out some or all of these forms in the past, you'll notice that they will look slightly different but contain the same information and function similarly. Email the ICT review team with questions or for additional help on this process.

OIT Project Forms (do not use for procurement reviews)

The following forms should only be submitted in order to complete the ICT Review process for an OIT Project.
