The following forms are to be completed by CU Requestors and Suppliers in the ICT Review process and ordering of any Information Communication Technology (ICT).
- Digital Technology Compliance Review Request (Form A)
- ICT Security Review (Form B)
- ICT Accessibility Review (Form C)

OIT Project Forms (do not use for procurement reviews)
