All intake with ADA Compliance begins with a form. Below you'll find the forms relevant to your situation, along with instructions for how to submit them. If you're unsure which form applies to you, contact us and we'll point you in the right direction. 

Employee Accommodation Request Form

This form is completed by the employee and provides ADA Compliance with an overview of the functional limitations related to your disability, as well as any specific accommodations you would like to request. Your Case Manager will review this form with you during your first meeting to clarify details and gather any additional information needed to move the process forward.

Medical Documentation Form

This form is completed by your medical provider and gives ADA Compliance the clinical information needed to assess your accommodation request — including your diagnosis, functional limitations, and any recommended accommodations. After meeting with you, your Case Manager may follow up directly with your provider if additional clarification is needed.

ASL / Captioning Request Form

This form is for event organizers and individuals requesting American Sign Language interpretation or real-time captioning for a campus event or public program. ADA Compliance coordinates scheduling with certified interpreters and captioning vendors on behalf of requesters. 

Please submit this form at least one week before your event — two weeks is preferred whenever possible. Requests received with less than one week's notice cannot be guaranteed, though we will always make every effort to accommodate late requests. 

How to Submit

Completed forms can be submitted to ADA Compliance by email at adacoordinator@colorado.edu or fax it to us at (303) 496-2330 in any file format. Medical documentation from a provider can also be submitted directly via fax or provider-direct submission — instructions for those methods are included on the Medical Documentation Form.
