To enroll in the public health certificate program, please fill out the enrollment form through the link below.

Note: You must use to access the form where "username" is your CU Login Name. If you are signed in to a different Google account while attempting to access the enrollment form, you will need to sign out of your other account and then sign in with your CU email account.

Enrollment Process

  1. The student will fill out the enrollment form above and submit it. (If you would like a confirmation of your form submission, please email
  2. The Administrative Assistant will process that form, and submit the certificate enrollment form to the Office of the Registrar.
  3. The Student and the Administrative Assistant will receive an email from the Office of the Registrar in 1-2 weeks* from the submission date. Once the Student receives this email, the certificate will show up on the Student's Degree Audit.
