A case investigator will be reaching out to you shortly to obtain a list of individuals who may have been affected and need to obtain testing for COVID. Note: Anyone notified as a close contact will only receive general information indicating that they may have been exposed. Your identity will not be disclosed.

You may email or call Health and Wellness Services for additional support: firstname.lastname@example.org or call 303-492-5101 and select “option 9” between the hours of 9 a.m. and 4 p.m. MT, Monday through Friday. We also have a 24/7 nurse line available for clinical consultation should your symptoms worsen.
STEP 3: Contact faculty members and (if applicable) work
Contact faculty members and work to let them know you will not be able to participate in in-person activities. If you have reached out to faculty and still aren’t sure what to do or if your symptoms become more acute and you are looking into additional academic options, please fill out the SSCM referral form and a Case Manager will reach out to you directly to provide assistance and/or connect you with additional support resources.
Steps for Release from Isolation
STEP 4: Receive additional information from Medical Services
You will receive additional information from Medical Services or your case investigator about the day you may be released from isolation. The day before your release from isolation, collect all of your belongings and prepare to vacate your isolation space.
