Step 2: Make an appointment and get tested
Make an appointment to get a free diagnostic test at Wardenburg by visiting mycuhealth.colorado.edu. You may also call 303-492-5101 and select option 1. After testing, go home and remain off campus until you have received the results of a diagnostic COVID-19 test.
If Wardenburg cannot perform the diagnostic test today, you need to get tested through your primary care physician or any other testing center. Tests performed outside of Wardenburg could delay test results and any additional instruction.
Step 3: Discuss your leave options with your supervisor or department HR.
You may be eligible to receive paid leave under the State of Colorado Healthy Families and Workplaces Act. The university will supplement an employee’s available sick leave balance, as necessary, to ensure that an employee may take 80 hours of sick leave (prorated based on percent of time).
If you already have 80 hours of sick leave accruals, you will not be granted any additional time.
Please complete the COVID-19 Leave and Accommodation Request Form and send the completed form to email@example.com to submit a request for sick leave. The request form and additional information about sick leave can be found on the HR Website.
Step 4: Provide test results to your supervisor
You will be required to provide the results of your diagnostic test to your supervisor and/or department Human Resources contact. Please provide this documentation as soon as possible.
If you receive a positive diagnostic test, you will receive any additional instructions from CU Public Health Clinic at Wardenburg.
If you receive a negative diagnostic test result, work with your supervisor to coordinate your return to work.
Guidance for Supervisors
Supervisors should ensure employees who have received a positive surveillance test notification go home and remain off campus until they have received the results of a diagnostic COVID-19 test. Employees will be required to provide the results of the diagnostic test to their supervisor or department HR Liaison.
**Supervisors should not take any additional action until they speak to an Employee Relations consultant. **
Supervisors will be contacted by Employee Relations to schedule an immediate meeting with Employee Relations to discuss next steps.
