Student COVID-19 Vaccine Requirement

To comply with CU Boulder’s COVID-19 vaccine requirement, members of the campus community are required to have received the initial series of vaccine plus one booster, or file for an exemption.

Students, faculty and staff must upload proof of boosters or file for an exemption based on medical, ethical or moral grounds through the MyCUHealth portal. Individuals who previously submitted an exemption do not need to take further action at this time and will be considered in compliance with current university vaccine requirements.

There are two ways to complete the vaccine requirement:

  • Proof of vaccination: Only COVID-19 vaccines that are approved by the World Health Organization (WHO) will meet the CU vaccine requirement.
  • Vaccine exemption: Personal exemptions will no longer be accepted. Only medical or moral/ethical exemptions are now allowed.

All students must complete the COVID-19 vaccine requirement or exemption in the MyCUHealth portal. The only exception will be students vaccinated through CU Boulder Medical Services. Data from Medical Services will be uploaded to the portal on behalf of students vaccinated through Medical Services

Submit proof of vaccination

To complete your vaccine requirements follow the steps below:
 

Step 1:

Go to the MyCUHealth portal, and select the option for Students and Graduate Students. Login with your CU Identikey and password.

Step 2:

Click Immunizations at top of the page. A drop-down menu will appear. Select Enter Dates from the drop-down menu.

Step 3:

Follow page instructions to add required vaccine dates and submit proof of vaccination.

Complete an exemption form

Faculty, staff and students who cannot or who choose not to get a vaccine can complete an exemption.

There are two types of exemptions:

  • Medical exemption: Medical exemptions must be signed by a licensed physician and must include a date and a reason for exemption.
  • Moral/ethical exemption: A moral/ethical exemption can be requested based on moral or ethical beliefs as to what is right or wrong that are held sincerely with the strength of traditional religious views.

Note: As with all required vaccines, in the event of an outbreak, exempted persons may be subject to exclusion from CU Boulder and quarantine. The specifics may vary depending on the circumstances of the outbreak.

 

Step 1:

Login to the MyCUHealth portal with your Identikey username and password.

Step 2:

Click on the “Immunizations” button at the top and from the drop-down menu select “Enter dates.”

Step 3:
Scroll through the text and select the link for a Medical or Non-Medical/Moral/Ethical exemption:

Medical exemption: Complete the medical exemption form with your medical provider. Once your form is completed and signed by a physician, login to the MyCUHealth portal. Click on the “Upload” button at the top and follow the instructions to upload your form.

Moral/Ethical exemption: Login to MyCUHealth portal. Click forms in the main menu. Scroll down to Health Services Forms and click Non Medical/Moral/Ethical Exemptions for vaccines. Fill out form and click submit.
