If you are a student with testing accommodations including 1.5x extended time or a distraction-reduced environment, faculty will provide your testing accommodations unless there is an extenuating circumstance.  Visit your professor during office hours or make an appointment to discuss your testing accommodations and how your professor will provide them.  If you have extenuating circumstances, contact your Disability Access Coordinator for assistance.

If you have more involved accommodations, such as double extended time or the use of a reader, scribe, or assistive technology, you can schedule your tests at Disability Services.  Complete the Proctor Request Form or Finals Proctor Request Form and email it to DSproctor@colorado.edu.  You will need to download and save the file to your desktop before entering data, then save the file again before submitting electronically. The form may also be dropped off at the Disability Services office.  To ensure that space and a proctor will be available, completed forms must be received 7 days in advance of your test.
