(Includes but is not limited to: Depressive Disorders, Post–Traumatic Stress Disorder, Bipolar Disorders, and Dissociative Disorders)

Students requesting support services from Disability Services are required to submit documentation of a disability to verify eligibility under the Americans with Disabilities Act (ADA), Section 504 of the Rehabilitation Act of 1973, and Disability Services’ policies. ADA defines a disability as a substantial limitation of a major life function. The diagnostic report must document a disability.

Submission of documentation is not the same as the request for services. Request for services and/or reasonable accommodations must be initiated by the student once he/she is confirmed at the University of Colorado at Boulder. The student must schedule an intake appointment with Disability Services so that support services and reasonable accommodations may be discussed. Documentation will be reviewed by Disability Services prior to the appointment. Reasonable accommodations cannot be implemented until the student’s documentation is complete. Disability Services is responsible for the determination of reasonable accommodations.

The following guidelines are provided to assist the diagnostician in providing the required information. Documentation must be submitted by a licensed clinical social worker (LCSW), licensed professional counselor (LPC), psychologist, psychiatrist, neurologist, or advanced practice/Psychiatric Mental Health Nurse Practitioner (PMHNP). The physician/therapist must be an impartial third party, not related by blood or marriage. Documentation should include the name, title, professional credential, license number, address, and phone number of the evaluator(s) as well as the date of the report.

  1. A clear statement of the disability, including the DSM–IV diagnosis and a summary of present symptoms;
  2. Documentation for eligibility should be current, preferably within the last three years; (the age of acceptable documentation is dependent upon the disabling condition, the current status of the student and the student's request for accommodations);
  3. A summary of assessment procedures and evaluation instruments used to make the diagnosis and a summary of evaluation results, including standardized or percentile scores;
  4. Medical information relating to the student's needs should include the impact of medication on the student's ability to meet the demands of the postsecondary environment;
  5. Suggestions of reasonable accommodations which might be appropriate at the postsecondary level are encouraged. These recommendations should be supported by the diagnosis.

Process for Requesting Accommodations

Students requesting support services and/or reasonable accommodations from the University of Colorado Boulder are required to submit documentation of a disabling condition to verify eligibility under the Americans with Disabilities Amendments Act, Section 504 of the Rehabilitation Act of 1973, and Disability Services’ policies. CU documentation requirements can be found here on this site. The following process is required for all students requesting accommodations and services:

Step 1

Submit a completed Application for Accommodations and Services form and your documentation of disability, which will be kept confidential. Complete documentation is important and inadequate information, incomplete answers and /or illegible handwriting will delay the eligibility review process and place additional burdens on the student. 

Step 2

Call our office at 303-492-8671 to schedule your intake appointment. Accommodations will not be provided until after the student has met with a Disability Specialist for an intake appointment. The process and procedures for using any approved accommodations will be discussed during this meeting.

Summer Intake for
Newly Admitted Students 
  Already on Campus
To schedule an intake appointment during your summer orientation call DS on the first day of orientation after you have received your orientation materials.  OR You may schedule your intake appointment when you submit your documentation. Intake appointments are scheduled at least five business days after you contact our office. 

Step 3

If approved for accommodations, you will be sent a letter, via email, to be given to your professors each semester. Print these letters and meet with each professor to discuss the arrangements for receiving your accommodations.
