To schedule final exams with Disability Services, fill out our Finals Proctor Request Form and submit to our office in the C4C building, suite N200, or by email to dsproctor@colorado.edu. Before submitting, please ensure both student and faculty portion of the Form are complete. The due date to have these Finals Proctor Request Forms in is Friday, April 5th.
