Procedures for Requesting Other Leave

  • Faculty member contacts the A&S Office of Faculty Affairs at CAS-Faculty@Colorado.edu and notifies the Faculty Affairs Coordinator that they are requesting a leave of absence. When contacting the office, please include the following information:
    • Chair’s name and email address (if rostered to more than one department, information for both chairs required);
    • Director’s name and email address (if rostered in an institute or museum);
    • Dates of leave request (either an entire semester or specific dates during a semester); and
    • Indicate if the leave is a LWOP, fellowship or other leave funded by another institution. If the leave is a LWOP, indicate if it is a leave to work in industry, a LWOP for personal reasons, or a LWOP due to a medical situation where all other leaves are exhausted.
  • Faculty members with split appointments MUST submit a leave of absence request from all units.
  • College of Arts and Sciences Faculty Affairs Coordinator reviews the submitted information and submits the Parental Leave Request for completion via Office of Faculty Affair’s Website.
    • The form is routed to the faculty member, chair(s) or director(s), dean(s) and the Office of Faculty Affairs for signature.
  • A copy of the final, approved Request for Leave of Absence is submitted by the power form to the following:
    • Unit(s), and
    • College of Arts and Sciences' Human Resources Service Center Technician.
  • College of Arts and Sciences' Human Resources Service Center enters leave and pay rate change (if applicable) in HCM (Human Resources Software).
