The Testing and Assessment Center uses the following forms. If you have other forms please contact us about getting them exchanged.
We provide three types of answer sheets:
5 Options, 120 Questions
- This answer sheet has space for up to 120 questions, with 5 options for each question. This form is purple.
- 5/120 Front View; 5/120 Back View
5 Options, 180 Questions
- This answer sheet has space for up to 180 questions, with 5 options for each question. There is also space on the front of the forms for essay responses. This form is blue.
- 5/180 Front View; 5/180 Back View
10 Options, 160 Questions
- This answer sheet has space for up to 160 questions, with 10 options for each question. This form is grey.
- 10/160 Front View
