Wardenburg Campus Care Plan

Exclusions

The following services are not covered under the Wardenburg Campus Care Plan. These include but are not limited to:

allergy antigens
birth control pills and devices
copies of X-rays and medical records
dental coverage
"Did Not Keep Appointment" (DKA) charges
equipment for home use
eye exams, contact lens fittings, or glasses
immunizations (except for sickness or injury)
knee braces
no benefits for children
no benefits for services outside Wardenburg Health Center, including when the health center is closed, if you are referred, or if Wardenburg Health Center is unable to provide you with the required care
orthotics
patient-requested cardiac risk profile, nutritional care, HIV testing, cholesterol testing, and hearing tests
premarital laboratory tests
prescription benefits
specialty services
sports treadmill
travel clinic and immunizations
psychological and psychiatry services
sports medicine/physical therapy services

Please keep in mind that no services outside of Wardenburg Health Center are covered through the Wardenburg Campus Care Plan if Wardenburg is closed.
