The Transcript Order Form must be completed in full and signed, or the order will not be accepted.
Students must present a valid photo ID when picking up a transcript order from the Office of the Registrar. All transcripts must be picked up within 30 days of the order. If someone other than the student will be picking up the transcript, the name of the person picking up the order must be stated on the order form. The person picking up the transcript must show a valid photo ID.
To comply with the Family Educational Rights and Privacy Act of 1974, all transcript requests must be made by the student; orders from third parties (including parents) will not be accepted without a valid Consent Form.
Official transcripts are withheld from students who have financial obligations and/or disciplinary actions in progress from any CU campus. Check the alerts section in MyCUInfo for any holds on your account.
If you intend to graduate in Spring 2015, complete each item below by the date listed. To find out about your graduation requirements, contact your department or advisor.
