To register for our programs please contact our office at (303) 492-6080 or email to cuop@colorado.edu This website will not register you for these trips.
Returnable Forms: ALL "Print Out and Return Promptly" forms must be completed and returned to us at your earliest convenience. All forms must be on file prior to your arrival.

Please fill out this registration form and send with payment to the CUOP

In the interest of personal safety for both the applicant and the other expedition members fill out the above physical form.

Complete, making sure to include necessary insurance information. Please make sure this form is signed by the participant, and by a parent/guardian if the participant is younger than 18 years old.

If you need a sleeping bag, backpack, or raingear, please let us know in the space provided. There is no charge for this equipment unless you lose or damage it while on CU Mountain Fest. Please check the appropriate space if you do not have any equipment needs.
