Step 6) Submit Health Records and Forms

Required Immunizations
All degree-seeking students born after January 1, 1957 must provide documentation for two rubeola (measles), two rubella (German measles), and two mumps immunizations. If you don't submit the documentation by the end of your first semester at CU-Boulder, a "stop" will be placed on your registration for the next semester. Immunizations are available at cost through Wardenburg Student Health Center on a walk-in basis. Medical, religious, and personal exemptions are allowed by law.

Submit a signed/stamped copy of your immunizations record (available from your physician, high school, previous university records, or military records) to Wardenburg Health Center, Immunization Office, University of Colorado at Boulder, 119 UCB, Boulder, CO 80309-0119; fax to 303-492-1014; or drop it off at Wardenburg 348.

Student Health Insurance
The University of Colorado endorses a policy of mandatory health insurance requiring all students on Boulder campus be covered by a comprehensive health insurance plan. Students may have coverage under their own individual, employer, parents, or spouse/domestic partner health plan.

However, all CU-Boulder students enrolled for one (1) or more credit hours are automatically enrolled and billed $650 for the Golden Buffalo Student Health Insurance "Student Gold" comprehensive plan unless they select the "Student Value" supplemental plan or waive the insurance by September 1, 2004 for Fall semester and January 19, 2005 for the Spring semester.
