Faculty Procedures
for Testing and Assessment
Please note the following
instructions to insure the classes’ exam is scored correctly:
1. Please use original scanner answer sheets only. Do not try to
duplicate scanner forms.
2. If you would like scores by class section number, please have the
students fill in their section # in columns labeled “1”, “2”, “3”, and
“4”. Since all 4 columns need to be filled out and recitations are 3
digits long, decide whether the students should fill the columns out:

Right-justified with
leading zero bubbled in column 1, or
-
Left-justified with a
trailing zero bubbled in column 4, or
-
You may separate the
sheets by sections into different piles.
3. Tests should be
submitted with an answer key filled out on a bubble sheet. If you have
more than one scale to be graded, submit an answer key for each scale
(maximum of 4 scales) PLUS one combined key with one response filled in
per question to serve as the master key.
4. If you have more than one correct answer to certain individual
questions, please submit additional keys each with an alternative answer
to those questions. If you use this option, please note that not as many
scales can be used. Although your keys have additional answers, student
responses are limited to one per question. Therefore, it is not possible
to design a question which instructs the student to answer “all possible
choices”, or “as many as are correct”. A separate key for each point
weight is necessary, and there is a maximum of four keys.
5. It is possible to give essay questions and have the score from those
questions added to the test. If you give essay questions, you may grade
these ahead of time and fill in the score in the CODE area as a whole
number. This score can then be added to the student’s total on the
objective section. Please fill in the full block (a score of 3, for
example, should be filled in as 003). This option reduces the number of
additional scales that may be used to three. A test may score to a
maximum of 999.99 points.
6. Please note that the scanner does not read letters. The circles must
be filled in completely, or the scanner will not read names, ID’s or
answers.
7. If you need to mark sheets for your own purposes, please use the very
bottom of the front page under the bubbles. If you need to mark any
other place, please use a red, green, or yellow magic marker with
caution.
8. Please collect the sheets so that all exams have the cut corner at
the same end (see bottom left hand corner).
