How do I get reimbursed for lab?

Gather all receipts together. If the receipts are smaller than 8 ½ X 11, then tape them onto a sheet of paper so they don’t get lost or separated.
Please fill out the attached form and submit it with the receipts. We’d appreciate it if you would fill it out on the computer before you print and sign it.
To complete the form, put:
1. Today’s date in upper right corner
2. The student’s name, address and phone under Payee information. (don’t worry about vendor ID or Fed ID).
3. Under description, put Jr. Lab (or Sr. Lab as appropriate), a hyphen, and where it was purchased from.
Example: Jr. Lab - McGuckins
4. Under amount, on that same line, put the amount of that particular purchase. Use as many lines as you need to list all of your purchases. The form will automatically total for you. Be sure you have a matching receipt attached for every line you fill in and the total is correct.
5. Print it (color is not necessary), sign and date on Requestor Signature line, attach all receipts, and bring it to the main office to turn in.
After we receive this form and the attached receipts, you should receive a check in the mail in a week or two.
