MS exam form |
If you are pursuing a Master's with thesis:
- Fill in personal information, test date, 3 examining committee members, and return/e-mail to the grad desk two weeks before exam. You don't need any committee sigatures yet.
- Pick the form up from the grad desk the day of the exam.
- After exam, have committee sign form and return it to the grad desk.
If you are pursuing a non-thesis Master's:
- Simply submit this form with signatures to the graduate desk any time after the submission of the final paper. It serves to show the Graduate School that you have the department's approval to receive your Master's.

| Final Grade Card |
You only need a final grade card if you have ungraded (in progress) dissertation credits. The final grade card is available at the physics grad desk, usually clipped to the Diploma Card.
- Your advisor (or titular advisor) will use the card to assign grades to your dissertation hours. Use the PLUS system to look up the semesters and credit hours for which you need grades.
- Give the card to your advisor the day of your defense.
- Remind your advisor to turn it in!
