Print out all these PDFs and give them to your Comps II committee chair. You committee will complete them and turn them in to the physics graduate program assistant. Forms are to be completed on the day of the examination.
Comps III Forms
What to do with them

Comps III form (see Graduate School website)

Fill in personal information, test date, examining committee members, and return/e-mail to the physics graduate program assistant two weeks before exam. You don't need any committee sigatures yet.

  1. Pick the form up from the physics graduate program assistant the day of the exam.
  2. After exam, have committe sign form and return it to the physics graduate program assistant.
  3. Bring a copy to your payroll person to get a raise.

Unsigned form due 2 weeks before exam.

Standard raise— $125/month. NOTE: You must be registered full-time during the semester in which you complete this exam.

Complete form.

  1. Get your advisor's signature.
  2. Remember to sign the last page yourself.
  3. Return form to physics office the day of your comps III exam.

Day of Final--NOTE: You must be registered full-time during the semester in which you complete this exam.

Diploma Card
  • Available from the physics graduate program assistant.
  • Complete and return to the physics graduate program assistant even if you do not plan to attend a ceremony.
  •      
    Final Grade Card
    1. Available from the physics graduate program assistant.
    2. Your advisor (or titular advisor) will use the card to assign grades to your dissertation hours. Use the CU Connect system to look up the semesters and credit hours for which you need grades.
    3. Give the card to your advisor the day of your defense.
    4. Remind your advisor to turn it in!

     

       

    Doctoral leaflet

    Complete and return or e-mail to the physics graduate program assistant.

     

     

       
    Dissertation
    1. Follow thesis specifications.
    2. Submit copies of your dissertation to the examining committee two weeks before exam.
    3. Submit final copy of dissertation to the Graduate school. See deadlines.

     

       

    Doctoral Exam Form

    Fill in personal information, test date, examining committee members, and return/e-mail to the physics graduate program assistant two weeks before exam (final defense deadline is noted at the right. Your form should be turned in two weeks prior.)
