Forms Initial Signup Form Initial Sign-Up Form to be filed with PACS Director Course Approval Form To be approved as a PACS student, complete this form with the help of a PACS faculty advisor in your (major) department or the PACS Program director. All courses on the "List of Approved Courses" may be used to meet requirements . Courses not on the list may also satisfy these requirements when approved in advance by a PACS faculty advisor or the PACS Program director. Certificate Completion Form Just prior to graduation, complete this form, attach a copy of your transcript, and submit them to your PACS faculty advisor. The transcript must include the 24 hours you have completed (from your Course Approval Form).
