                              COURSE APPROVAL FORM

To be approved as a PACS student, complete this form with the help of a PACS
faculty advisor in your (major) department or the PACS
Program director.  All courses on the "List of Approved Courses" may be
used to meet requirements 2B and 2C (listed below). Courses not on the
list may also satisfy these requirements when approved in advance by a
PACS faculty advisor or the PACS Program director(s).

                                 REQUIREMENTS:

1. Completion of a major in any discipline in the University of Colorado; graduation from CU.
  To obtain certificate without a CU degree, please contact Dylan Clark.

2. 24 hours of CU course work (C or better) 15 of which must be upper division.

3. Of the 24 credit hours:
     A. PACS 2500 (Introduction) and PACS 4500 (Senior Seminar)must be completed.
     B. 9 hours must be in the student's major.*
     C. 9 hours must be outside the student's major.

     *Should a student's major department not have 9 hours of relevant
      courses, a PACS faculty advisor may permit substitution from the PACS list. 


You and your PACS faculty advisor in your major department should each keep a
copy of this form while you file the original with the PACS Program director(s):
