The following documentation shows the process of creating a grade item from the Grades section of Desire2Learn. This will show the process of creating a grade item before creating the assignment it is attached to. Creating a grade item is essentially adding a new column in the grade book once attached to an assignment or quiz.
Grade items can also be created while creating an assignment, quiz or discussion item you would like to be graded. When creating these items there is an [add grade item] link that opens a new window. This window will show the options shown in steps 4-6, so reference those steps when simultaneously creating a grade item and assignment or quiz.
Log into your course and from the Assessments drop-down menu click Grades.
Choose the type of grade item you would like. The most common is Numeric as it is simply a number of points received out of points possible (ex. 45/50).

Choose the options that you would like for your grade item. Required fields are Name: and Max Points:.
Other fields include:
Define whether or not you want to limit the times that an item is available for student viewing.
Also, if desired, attach release conditions for an item (ex. once item A has been submitted, item B can then be seen by a student)
Click Save and Close. Now this grade item can be assigned to an assignment, discussion, quiz, or assessment of any kind.
