Great! Your protocol has been approved. Now what? There are some ongoing responsibilities for each Principle Investigator once your protocol has been approved. It is quite simple to maintain approval of your protocol.
Two months prior to the first anniversary of your protocol, you must submit an Annual Update Form. Update the IACUC Office on your animal use for the past year, and give us an idea of your planned use for the next year. Fill out this form and email to the IACUC Office. You will not necessarily receive a reminder to do so.
Two months prior to the second anniversary of your protocol, you must submit an Annual Update Form. Update the IACUC Office on your animal use for the past year, and give us an idea of your planned use for the final year of your protocol. Fill out this form and email to the IACUC Office. You will not necessarily receive a reminder to do so.
Two months prior to the expiration date of your protocol (or earlier) send us the Three Year Update Form. This tells the IACUC Office whether you plan to renew your protocol or close your protocol. If you are renewing your protocol, also send us your revised Protocol Application. Remember, all addenda you’ve accumulated on your protocol over the past three years expire with the protocol, so make sure you embed the information in your addenda into the renewal Protocol Application if you wish to continue those modifications you’ve made.
Report all unanticipated/adverse events that occur on your protocol. The investigator will notify the IACUC Office (303-492-4787) of any unexpected results that adversely affect the welfare of the animals. Investigator will report any unanticipated pain or distress, morbidity or mortality to the attending veterinarian and the IACUC. If there is an adverse event involving the health and/or safety of any research or animal handling personnel, the investigator will contact the IACUC and EH&S (303 492-7072), and complete a University Risk Management Employee's Injury Report Form within 48 hours of the event. Investigator will also provide the IACUC (UCB 345) and EH&S (UCB 413) with a copy of this completed form.

If you order animals from vendors, you must check with the IACUC Office as to whether you have enough animals remaining on your protocol BEFORE you place the order. The IACUC Office turn-around time is less than 48 hours. Email firstname.lastname@example.org. If you breed animals here on campus, you must contact the IACUC Office at the end of each month with the number of animals “weaned” in the past month, so we can deduct from your protocol. This system is for the interim until we get an electronic database for the animal program, or we move to central ordering.
