Dear <insert employee name>: The University of Colorado at Boulder, as your employer, is required by federal law to ensure proper tax withholding from wages and to ensure that the required reporting of employee wages, withholding, and employment taxes is accurate. For this reason, every employee must provide a copy of his or her social security or tax identification card to the department in which he/she was hired. If you do not have a social security number, you are required to apply for one from the Social Security Administration. The local Social Security Administration office is located in Boulder at 665 S. Broadway. You must provide a copy of your new social security card to <insert name of department representative> within 30 days of employment. Important note: Failure to provide a copy of your social security card within the allotted time period may result in termination of employment with the University.
