New Employee Checklist

This list explains what information or documents a new employee should bring to their first day of work.

Documents/Paperwork Comments and Policies
Original Social Security Card Laminated or faxed copies are not valid. http://www.cusys.edu/policies/Fiscal/socialsec.html
Voided check or saving account deposit slip for Direct Deposit Employees are allowed to split their direct deposit between 3 different accounts. http://www.cusys.edu/policies/Fiscal/payroll.html
Acceptable documents for I9 The university is required by federal law to verify each new employee's eligibility to work in the United States within three days of university employment. You must provide appropriate documentation to verify your eligibility. Click here for more I-9 information.
Emergency contact information Emergency Contact Data contains the names, addresses, and telephone numbers of the people to contact in the event of an employee emergency.
Beneficiaries name and social security numbers Retirement accounts, PERA or TIAA-CREF, have a place to designate beneficiaries.
SSA-1945 Form This form should have been mailed to you with your letter of offer if you are Classified Staff.
