Required health forms
Effective June 2, 2008, you can enter your immunization information online through OSH >> The medical history form can no longer be printed, only completed through OSH >> You must have an IdentiKey login and password to access OSH.
For your convenience, required health forms are also available online in PDF format for you to print. Click on a link below, then chose "File," and "Save As" to save to your computer. Please print on white paper and use black ink.
Forms (online or paper) must be completed and received by semester deadlines to avoid a late fee and hold on your student account.

- Authorization for Treatment of a Minor Form - completion only required if student is under the age of 18 when arriving on campus
** All new students are required to complete and submit these three forms.
Deadline for submission
Mandatory forms from all new students must be received by the semester deadline according to their first semester of classes at CU-Boulder or a $25 fee will be assessed and a hold will be placed on the students' accounts preventing them from registering for any following semester.
