Immunizations
Colorado State Law requires new freshman, transfer, and graduate students to submit proof of two doses each of measles (rubeola), two rubella (German measles), and two mumps (or "MMR") vaccinations. This is a mandatory requirement for all college students born on or after January 1, 1957.

- Authorization for Treatment of a Minor Form - completion only required if student is under the age of 18 when arriving on campus
** All new students are required to complete and submit these three forms.
Deadline for submission
Mandatory forms from all new students must be received by the semester deadline according to their first semester of classes at CU-Boulder or a $25 fee will be assessed and a hold will be placed on the students' accounts preventing them from registering for any following semester.
