Health Records and Confidentiality

A confidential health record is created for you after your first visit to the Wardenburg. Health information cannot be released without your written consent unless mandated by law. (If you are under 18 years of age, your parent(s) or legal guardian(s) must sign a Consent for Treatment Form, which is kept in your health record file.) Medical and mental health records are not included in the general university record system. Health records are maintained in compliance with federal and Colorado laws and are destroyed in a confidential manner after 10 years.

Prior Medical History

If an incoming student has chronic medical conditions, we recommend their doctor's office send a copy of pertinent medical records to Wardenburg Health Center. 

Additionally, be sure to complete the Medical History form online through Online Student Health

 

Release of Information

A Release of Information form is available if you would like to release your medical information. This form must be filled out in person at Wardenburg, or contact us regarding faxing a release form in.
