Appointment Procedure
How to make an appointment for this clinic:
Call 303.492.5654, M – F: 8AM – 5PM to schedule an appointment. If this is your first visit with PHP, you will need to come into the clinic to fill out necessary paperwork (about 15 minutes) before scheduling your appointment.
Cancellation Procedure:
Appointments must be cancelled by 5PM the business day before your appointment by calling 303.492.5654.
Missed appointment or late cancel/reschedule fees:
- Therapist appointments - $30
- Psychiatrist appointments - Initial (first) visit: $80, follow-up visits: $30
- ADHD testing - $80
Check-in Procedure:
Please arrive 15 – 30 minutes before your appointment to fill out any necessary paperwork. Self check-in computers are located on the first floor in the PHP waiting area. *Arriving more than 10 minutes late may result in your being rescheduled.
