About Online Student Health, or "OSH"
What is Online Student Health - OSH?
OSH is a system that works with Wardenburg's electronic medical records system that allows you to schedule appointments online (for some services), complete paperwork online, and communicate with your health care providers through secure messaging. OSH's secure messaging system is more confidential than e-mail. We may use this to send messages to you, and you can use it to send messages to us
Who can use OSH?
Any CU student with an IdentiKey and IdentiKey password can use OSH. Spouses/SGDPs and dependents of students who are enrolled in the Student Health Plans, faculty and staff, camps and conferences participants, and campus visitors cannot use OSH.
New students: we may not have you name in our system yet if the registrar hasn't transferred it. If this is the case, please contact us by phone to make an appointment, print your forms to submit, or wait until later in the summer to access OSH.

Health Forms Submission
Which forms can I complete through Online Student Health - OSH?
- Certificate of Immunization for College Students Form **
- Medical History Form - required only if you plan to use services at Wardenburg
** All new students are required to complete and submit this form.
Eventually, we hope to have all three required forms available to complete online.
Which forms are available as paper copies only?
- Mandatory Meningococcal Disease Information Form **
- Tuberculosis Information and Screening Form **
** All new students are required to complete and submit this form.
The Authorization for Treatment of a Minor form (required only if you will be under the age of 18 when arriving on campus) cannot be completed online. You must submit a paper copy via snail mail, fax, or in person (print form).
If I submit my immunization record online, do I still need to mail the paper immunizations form?
Yes - submitting immunizations online can help you avoid a late fee and a registration stop, but we need to see the health provider’s signature stating that your immunization record is correct. Until we receive this, your immunization record is not valid.
Do I have to to complete my health forms online?
No - but when we eventually have all the forms available online we will prefer you do as it will save paper and we don't have to worry about illegible hand-writing. But, you can still print forms to paper and submit them via snail mail, fax, or in person.
The Medical History form must be completed online prior to your first appointment at Wardenburg.
Online Scheduling
What type of appointments can I schedule through Online Student Health - OSH?
Only the following appointments can be scheduled through OSH.
For any other appointment, you must call to schedule.
- Medical Clinic
- allergy shots
- rash
- physical
- warts (extremities only; not genital)
- certain respiratory symptoms/illnesses:
- bronchitis
- cold
- congestion
- earache
- mono
- respiratory flu (not "stomach flu")
- sore throat
- Sports Medicine
- acupuncture
- massage
- Women’s Health Services
- annual exam
- birth control consultation
All Psychological Health and Psychiatry appointments must be scheduled by calling 303.492.5654.
Free HIV testing does not require an appointment and is offered through Community Health at the UMC.
Is there anything else I need to do before I come in for my appointment?
Yes - prior to your first appointment at Wardenburg you must complete the Medical History form online through OSH.
Also, save yourself time by completing your appointment-specific check-in forms through OSH before you come in (how long you have had symptoms).
You will still need to swipe your Buff OneCard at a Wardenburg check-in computer when you arrive for your visit. Remember to always arrive 15 minutes before your scheduled appointment.
Can I schedule a same-day appointment online?
Yes - some same-day appointments for the Medical Clinic can be scheduled online. At 5PM, you will be able to schedule an appointment for the following morning. At 8AM, you will be able to schedule an appointment any time that day. Appointments must follow the criteria listed in the question above.
Why isn’t the system finding any available appointments?
Try expanding your search parameters. Some providers, such as our massage therapists, are only here on certain days.

Can I cancel an appointment online?
No - appointements must be canceled by phone or in person by 5PM on the business day before your appointment. Appointments cannot be canceled via e-mail or OSH.
