A confidential health record is created for you after your first visit to the Wardenburg. Health information cannot be released without your written consent unless mandated by law. (If you are under 18 years of age, your parent(s) or legal guardian(s) must sign a Consent for Treatment Form, which is kept in your health record file.) Medical and mental health records are not included in the general university record system. Health records are maintained in compliance with federal and Colorado retention regulations and are destroyed in a confidential and secure manner.
