COPFFN Fellow Mentorship Plan. The student, employer, and the Graduate Teacher Program agree to assume these responsibilities for the duration of the student's placement. The Mentorship Plan must be completed in order for the Graduate Teacher Program to endorse the student's placement and for the placement to be considered "sponsored" by the University of Colorado at Boulder.
This form tracks GTP workshop attendance and discipline-specific training hours. It is used for tracking workshops for the certificate in college teaching and for lead graduate teacher application tracking.
If your department identifies you as the candidate as the Lead Graduate Teacher, your department chair, your academic advisor, your TA supervisor/coordinator (if applicable), and your Lead faculty mentor, must all sign this permission form.
