survey focuses on your organization’s activities and interests. We ask you to
list information about your organization’s goals, activities, and
characteristics. In addition, this survey asks about some of your personal
views and opinions regarding organizations in general.

We
recognize that some questions are private in nature. Please be assured that your responses will
be held strictly in confidence and your organization will not be identified
by name in the final report. Your
responses will be seen only by the research investigators and will not be
distributed to any other unauthorized individual. For the purpose or tracking
survey responses, we ask that you identify the organization for which you are
providing answers. Finally, please submit only one survey per organizations.
