Each item is listed with a number--this is the number to be recorded on the Optional Question Request Form (or Group Optional Request Form, for departments) to have that item printed on a set of FCQ forms. You can search for optional questions you have used in the past 5 years. You can also search optional questions for particular text strings. You may request your Optional Questions via the World-Wide Web (Click Here to do so.)
