The following screens list over 750 items that have been used as optional questions on the Faculty Course Questionnaire (FCQ) in the last few years. Each item is listed with a number--this is the number to be recorded on the Optional Question Request Form (or Group Optional Request Form, for departments) to have that item printed on a set of FCQ forms.
