Instructions for Administrators

FCQ administration week is the week before the last week of classes. Do not use finals
to administer FCQs.
Check to make sure that the department, course, and section numbers printed on the FCQ forms
correspond to this class. If they do not, please see your department’s FCQ coordinator
immediately or call the FCQ office at 303-492-7069. If there is no time for this, mark
corrections clearly on the envelope, near the label, and explain the problem.
Do not use photocopies or forms printed for another class!

What to Do
- Pass out one form to each student. READ ALOUD as students read along the instructions
on the FCQ form, side A:
FCQ results are used by students in selecting courses, by instructors to improve their
teaching, and by administrators for tenure and promotion decisions.
Neither instructors nor TAs for the course may be present at any time during administration of this form.
These forms will not be returned to your instructor until after grades have been posted.
Note: If multiple forms are determined to be from one person, they will be removed from processing.
If a form contains abusive comments, it may also be removed.
- Remind students to answer questions on Side B (if any)
- Remind students to use a No. 2 pencil only!
- Note: If this is a recitation or lab section, remind students they are to evaluate
their lab or recitation at this time. They will be given the opportunity to evaluate the
lecture and the lecturer on a separate evaluation.
- Please allow students 15 minutes to complete
the FCQ form. When everyone is finished, collect the FCQ forms and place
in the FCQ envelope along with any unused forms. Be sure
each FCQ form is placed in the FCQ envelope to which it belongs if there
are multiple packets of FCQ forms being administered in the same classroom.
Return the envelope IMMEDIATELY to your department office; or hand deliver to the FCQ office,
Regent 1B29; or send by campus mail to Campus Box 15 (last two options for Boulder campus only.)
Do not return FCQ forms to the instructor. The instructor is not to handle the FCQ forms in any
way after administration.

Faculty Course Questionnaire (FCQ)

FCQ administration week is the week before the last week of classes. Do not use
finals to administer FCQs.
The printed label or handwritten information on the front of this envelope lists the course
subject, course number and section. Please check to make sure that you have the correct forms
and the correct number of forms for your class. If the information is incorrect, contact your
department FCQ coordinator immediately to have another packet generated, or call the FCQ
office at 303-492-7069. If there is no time for this, mark corrections clearly on the
envelope, near the label, and explain the problem.

To eliminate the possibility that students may complete more than one form for each course, only
one administration per course is permitted. Students may NOT take away forms to be returned at a
later date, nor may some of the forms be administered during a subsequent class.
How the forms are administered is as important as the questions. Please follow these procedures:
- ANNOUNCE the date you plan to administer the FCQ form.
- Make sure No. 2 pencils will be available on the administration date.
- On administration day, give the packet to a responsible person for administration. LEAVE THE
ROOM for the duration of the course evaluation. NEITHER INSTRUCTORS NOR TAS FOR THIS CLASS MAY
BE PRESENT at any time during the administration. This is an extremely sensitive issue to
students. Please respect the students' rights and scrupulously adhere to the spirit and intent
of this requirement.
- Please allow the students AT LEAST FIFTEEN MINUTES TO COMPLETE THE FCQ. This should
allow enough time for students who want to offer comments.
- The administrator,
not the instructor, is to return all forms in the original envelope
to the department office; or to the FCQ office (Regent 1B29);
or by campus mail to Campus Box 15 (Boulder campus). All Denver
and Colorado Springs envelopes are to be returned to their department
office only. The instructor may not collect the forms or bring
them back to the department office or to the FCQ office. This is another
sensitive area for students. The instructor is not to handle the FCQ forms in any way after
administration.
- Of special note to recitation and
lab section leaders: If the FCQ packet is for a recitation or lab, please
be sure the administrator tells students they are evaluating the recitation
leader and the recitation itself; they will have an opportunity to evaluate
the lecture on a separate evaluation.
The original forms will be returned to your
department, along with a report of statistical results, usually 4-6 weeks
after the end of the semester.

Background on the FCQ
The Faculty Course Questionnaire program
allows students to rate their instructors and courses at the end
of each term. It provides information to instructors for teaching
improvement, to students for course selection, and to deans and
chairs for management. Evaluation through the FCQ is available to
instructors of any type of section -- lecture, seminar, lab, recitation
-- regardless of status (faculty, teaching assistants, other instructors).
There is no charge to individual instructors. See your dean or chair
for information on who must participate.
