Dear Student:
The deadline to turn in immunization records was February 04,2011. We extended it to February 18, 2011. An immunization Hold has been placed on your registration for Summer/Fall and a $25 late fee assessed.
The State of Colorado requires all new students, born Jan 1, 1957 or after, to submit proof of two doses of Measles, Mumps and Rubella vaccines (MMR). You have either not turned in records or the records you have turned in are not complete.
Please contact the Immunization Office by phone or in person to verify what is needed for compliancy, at (303) 492-2005. Immunization forms can be downloaded from our website at http://healthcenter.colorado.edu/forms/immunizations/. Please fax your records to 303-492-1014 or bring to our office in Wardenburg Health Center, room 333.
