Dear Student,
A stop (negative service indicator) has been placed on your registration as the university's records indicate you have not complied with immunization requirements. Until you clear this stop with Wardenburg Health Center, you will not be able to register for Summer or Fall 2011 courses.
A fee assessment of $25 will also need to be paid before the stop will be cleared. This must be paid at Wardenberg Business office room 251 or at the Bursar's Office located in Regent Hall room 150. For information on this stop, call the Wardenburg Immunization Office at 303 492 2005 or appear in person to Wardenburg Student Health Center Room 333.

Print out this form and have your doctor sign it or attach signed records of immunization and FAX it to the Immunization Office in Wardenburg (number above) or take it to Wardenburg Room 333. The web site will provide immunization information on what is required, what is strongly recommended and general information.
NOTE: If you have other stops and/or advising requirements you must also clear these before you'll be eligible to register. Be sure to clear all stops at least 24 hours before your registration time assignment. Summer registration starts March 2nd.
You can obtain information about any other stops on your record by accessing your registration eligibility for the coming semester via the Web, go to MyCUInfo at https://portal.cusys.edu/MyCUInfoFedAuthLogin.html. You will need your identikey and password to do this.
