SUBJECT: You have been enrolled in the Student Gold Insurance plan
You are currently enrolled in the Student Health Gold Insurance plan by default. The insurance is effective from August 18, 2010 - December 31, 2010. Your tuition bill has been charged $1,105.00 for the Fall 2010 semester premium. You should have received an insurance card and plan booklet from the underwriting insurance company, United HealthCare Student Resources. Please be familiar with the booklet since it describes your health insurance benefits for care provided outside of Wardenburg Health Center.
If you do not want to coverage under the Student Gold Plan, please come to Wardenburg Health Center, room 336, and complete a waiver form no later than October 29, 2010. Due to the University's mandatory insurance coverage policy, you must provide proof of insurance in order to waive the Gold Plan.
