SUBJECT: Wardenburg Campus Care Enrollment
Dear Student,
You are enrolled in the Wardenburg Campus Care Plan ($150.00 per semester) for
the spring 2008 semester. Your coverage for the spring 2008 semester will
start on January 5, 2008. This only covers primary care (general or family
practitioner visits), medically necessary labs and x-rays, and women's health
services inside Wardenburg Health Center. It does not cover specialists,
specialty procedures, prescriptions, physical therapy, psychotherapy, or
anything outside of Wardenburg Health Center (Please see the Wardenburg Campus
Care brochure for more exceptions that are not covered).
If you would like to change your selection to the Student Gold Plan ($790.00
per semester) or waive the student health plans completely your deadline to do
so is January 23, 2008. The change can be done one of two ways. You can
either fill out a selection/waiver form in the Student Health Plans office
inside Wardenburg. Or you can go online through CUConnect. Once you log on,
go under the financial tab, click on the -view enrollment- button, and then
click on the -change selection- button.
