IMPORTANT ANNOUNCEMENT REGARDING
MEDICAL TREATMENT
FOR ON-THE-JOB INJURIES
This is to notify all CU-Boulder employees of the final selection of the two permanent Designated Medical Providers for treatment of on-the-job injuries. Effective immediately, employees may choose one of the following Providers for treatment of new injuries:
Arbor Occupational Medicine, 4790 Table Mesa Drive, Suite 200, Boulder. Phone: (303)443-0496
WORKWELL Occupational Medicine, Burlington Medical Center, 205 S. Main St., Suite C, Longmont. Phone: (303)702-1612.
